Welcome to the Community Hospice Foundation website. Our Wisconsin based hospice Foundation is a charitable, non-profit 501(c)(3) organization dedicated to providing financial assistance to improve the quality of life of those who have been diagnosed with a terminal illness. The Foundation is funded by generous donations from grateful families and friends of patients who have been served by ThedaCare At Home™. Community Hospice Foundation is devoted solely to funding compassionate caring programs for people at end-of-life as well as supportive services for their families.
