Through Community Hospice Foundation, ThedaCare At Home™ helps to raise awareness and the funds needed to provide comfort and support to patients and families at the end-of-life. Your contribution goes directly toward meeting the specific care needs of adults and children in the communities we serve. Your gifts can help us reach out to those communities with professional education and community outreach programs that help ensure everyone in our community knows more about end-of-life issues and has access to our services.
With your gift, you can provide the following:
- Care for patients who do not have insurance or have hospice benefits in their plan.
- Education, outreach and bereavement services.
- Facilities improvements at ThedaCare At Home’s hospice inpatient centers.
- Life enhancing therapies for patients at end-of-life.
- A “Wish” for a patient or their family through the Hopes & Dreams Fund.
There are many opportunities to support the important work of the Community Hospice Foundation.
When you make a gift to the Community Hospice Foundation you have the flexibility to determine how your gift is used and how your gift is delivered—for example, through a cash gift, gift of stock, event sponsorship, or by will.
