The Community Hospice Foundation, is the philanthropic arm for ThedaCare At Home’s™ Hospice program.  The Foundation is dedicated to supporting programs and services that enhance the quality of life for individuals with a life-limiting illness.  The funds raised through donations and special community events allow ThedaCare At Home™ to continue to provide a high standard of excellence in the programs and social services it provides to the community.

How the Community Hospice Foundation supports hospice care…

Hospice care differs from other forms of health care because it addresses not only the needs of the patient, but also the needs of the family and other primary caregivers.  Hospice professionals help patients and families deal with the significant medical, emotional, psychological and spiritual changes in their lives. When the patient experiences the natural course of the disease process, his or her life is enhanced with compassion and dignity through hospice care.  That’s where Community Hospice Foundation comes in.  We provide special grants and funding for individuals who need emotional, medical or financial support to care for their loved one.
